Upon receiving written orders, initiates the patient care process of assessment of the patient's functional status and needs. This patient care process includes, but is not limited to:Formulation of patient diagnosis and prognosis based on evaluative findings and interpretation of these findingsDevelopment of an appropriate and specific treatment plan of care, complete with measurable, realistic, and timely goalsReassessment of progress as needed based on established timeframes, patient response to treatment, or change in statusFormulation of discharge plan and ability to work with patient, patient family members, and other healthcare team members to successfully carry out discharge planDocumentation of patient evaluation, progress notes, re-evaluations, and discharge in a thoroughly and timely manner in the patient medical record2. Actively participates in walk rounds, verbal rounds and patient care team conferences when appropriate.3. Prepares daily patient charge records of treatments and supplies.4. Participates in the Clinical Education Program through the supervision and instruction of affiliating Physical Therapy students and interns.5. Supervises/Directs Physical Therapy Aides and Physical Therapy Assistants in performance of assigned tasks, when indicated.6. Participates in staff meetings, departmental groups aimed at improving clinical practice, and in-service education programs both inside and outside of the institution.7. Demonstrates interpersonal and communication skills necessary for success in a multi-disciplinary, acute care hospital setting.8. Completes annual safety testing and departmental competencies.9. New grads welcomePay Range$39.50 - $55.31The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
