Community Health Navigator

Breakthrough

Wellness West is seeking a Community Health Worker to serve on Chicago’s west side, connecting patients with medical care and local resources. You will maintain a steady caseload, perform health assessments, coordinate referrals, and educate community members about primary care and social determinants of health. The role requires occasional evenings/weekends and works onsite at multiple centers in Chicago. #J-18808-Ljbffr Breakthrough
