Position Overview Metro Community Health Centers is seeking a Family Nurse Practitioner (FNP) to provide high-quality primary care services to patients of all ages. The FNP will work collaboratively with physicians and a multidisciplinary care team to deliver patient-centered care to the communities we serve. This is an excellent opportunity for a provider passionate about community health, preventive care, and serving underserved populations. Responsibilities Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions. Perform physical exams and obtain detailed patient histories. Order and interpret laboratory tests and diagnostic studies. Prescribe medications and develop treatment plans in accordance with NYS scope of practice. Provide preventive care including health screenings, immunizations, and patient education. Coordinate care with physicians, specialists, behavioral health providers, and care teams. Maintain accurate documentation in the electronic health record (EHR). Participate in quality improvement and population health initiatives. Ensure compliance with all federal, state, and organizational policies. Qualifications Master’s Degree in Nursing (MSN) from an accredited Nurse Practitioner program. Current New York State Nurse Practitioner license. Board Certification as a Family Nurse Practitioner (FNP). DEA license or ability to obtain. Experience in primary care, community health, or FQHC settings preferred. Strong clinical, communication, and patient engagement skills. Experience with electronic health records (eClinicalWorks preferred). Bilingual skills a plus. Why Join MCHC?
