Job Title: Home Care Coordinator Location: Wallingford, CT — On-site Type: Full-Time | On-site Positions Available: 2 Job Summary We are seeking a Home Care Coordinator to manage scheduling, intake, and care coordination for a senior living home care program in Connecticut. The coordinator will serve as the primary point of contact for patients, families, referral sources, and field caregiving staff. Key Responsibilities Manage intake and onboarding of new home care patients and clients Schedule and coordinate home care visits for nursing and aide staff Serve as primary liaison between patients, families, referral sources, and care team Monitor care delivery compliance and follow up on missed or incomplete visits Maintain accurate records in home care management software Support billing, authorizations, and insurance verification processes as needed Requirements 2+ years of experience in home care coordination, scheduling, or patient services Strong knowledge of home care operations and Medicare/Medicaid requirements preferred Excellent organizational, communication, and problem-solving skills Experience with home care scheduling software preferred Compassionate approach to working with seniors and their families #J-18808-Ljbffr
