Family Transitions, Inc. is looking for Qualified Mental Health Professionals (QMHP-C) to provide
intensive in-home services.

Responsibilities and Duties:

Provide mental health treatment to individuals receiving services (services may include Intensive
In-Home, through individual and family therapy, behavior management, care coordination, crisis intervention, psycho-education, and skills training.
Attend staff meetings and trainings as required (supervision is provided weekly and trainings are
at least monthly)
Collaborate with referral source and other treatment providers
Create individualized service plans (ISPs) and implement the goals and objectives
Complete required clinical documentation
Ensure compliance with regulations set by governing bodies and maintain standards of care
according to Family Transitions, Inc. policies and procedures.
Complete daily progress notes at minimum on a weekly basis by 9:00AM on Monday morning.
Certifications/Requirements:
Qualified Mental Health Professionals-C (QMHP-Cs): Must be registered with the Board of
Health Professions as a Qualified Mental Health Professional for Children. If not registered, must
have at least a Bachelors degree in a human service field and 1 year of full-time experience.
Residents in Counseling: Must have a Masters degree in Counseling with 500 internship hours or
1 year of experience. Supervisees in Social Work: Must have a Masters degree in Social Work with 500 internship
hours or 1 year of experience. Salary:
Compensation is competitive and based on experience. Currently offering 23.00 to 27.00 per
hour.
Bonuses based on performance
Clinical supervision for Licensed Professional Counselors (LPC CEU opportunities for trainings of
interest)
Flexible Work Schedule - Early evening and weekend hours available Possible Position advancement
