Legacy Community Health seeks a compassionate RN – High Risk OB Care Manager to join its maternal health team in Houston. This role focuses on high-risk perinatal care coordination, transitions-of-care, and triage for pregnant and postpartum individuals with complex medical, behavioral health, or social needs. You will work with physicians, APs, Care Management, and Perinatal Navigators to navigate complex care pathways, aiming to reduce obstetric complications and improve postpartum follow-up #J-18808-Ljbffr Legacy Community Health
