Case Manager - Acute Care

Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30). Must work every other weekend.

Job Description

We are seeking an experienced Acute Care Case Manager to join our healthcare team in Eureka, California. This role involves managing patient care efficiently while coordinating with interdisciplinary teams to ensure optimal patient outcomes.

Responsibilities

• Coordinate patient care from admission to discharge.
• Conduct assessments, develop care plans, and facilitate communication among healthcare providers.
• Manage appeals and denials, ensuring compliance with CMS regulations.
• Assist with discharge planning and resource allocation.
• Support patients and families in understanding benefits eligibility and advance directives.
• Participate in quality improvement initiatives and adhere to Joint Commission standards.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care case management experience.
• Experience with HMOs, IPAs, and protective service processes.
• Current RN license or applicable state licensure.
• Familiarity with Epic charting system is required.
• Strong understanding of Medicare guidelines and discharge planning.
