Requirements
- Active Registered Nurse (RN) license
- Experience with Residex/RTask

Roles & Responsibilities
- Coordinate patient care and services to ensure continuity of care.
- Develop and implement individualized care plans for patients.
- Collaborate with multidisciplinary teams to enhance patient outcomes.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and families on care plans and healthcare resources.
- Document patient interactions and care activities accurately.
