Coverage Territory: Douglas, Edgar, Macon, Cumberland, Shelby and surrounding counties

Schedule: 8 AM8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $70,000 - $85,000 a year

What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:

• Delivering professional nursing care across the full nursing process within your scope of practice
• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing all visit types as needed to meet patient and family needs
• Administering medications and treatments per physician orders
• Identifying nursing diagnoses from health assessment data and responding accordingly
• Recording accurate, timely clinical notes including pain and symptom management outcomes
• Communicating changes in patient condition to physicians and obtaining orders as required
• Coordinating with community health partners to support seamless care transitions
• Ensuring equipment and necessary services are arranged and available for patients
• Maintaining and growing your clinical skills through continuing education and in-service participation

Requirements

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred
• Flexibility and confidence working across varied patient assignments
• Strong assessment skills and sound clinical judgment under pressure
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally
