Physical Therapist Resident Care & Discharge Planning
Benedictine Health System
St. Gabriel's Community in New Brighton, MN is seeking a dedicated Physical Therapist to evaluate residents, carry out ongoing assessments, and implement therapy interventions.
The role involves developing and revising care plans, setting functional goals, planning discharges, and educating the care team, residents, families, and caregivers on the plan of care. A state license in Physical Therapy is required and a Bachelor’s degree is essential.
