This position is centered around ASAP consumer care, requiring strong knowledge of community-based services, compliance, and supervisory oversight.

About the Role

We are seeking an experienced RN or LPN for a full-time, field-based role. This position requires daily travel to client homes to complete supervisory visits, assessments, and ongoing care management. You will be working independently in the field while collaborating closely with case managers, families, and our internal team.

Compensation & Benefits

Salary: $90,000 $105,000 annually (based on experience)Mileage reimbursement providedFull-time, stable MondayFriday schedule

Key Responsibilities

Complete in-home supervisory visits for HHA/PCA caregiversConduct client assessments and reassessmentsEnsure compliance with ASAP program requirementsCommunicate with ASAP case managers and familiesMonitor client conditions and report changesMaintain accurate and timely documentation

Requirements

Active RN or LPN license in MassachusettsMinimum 1 year of home care experience preferred, but not required (new grads encouraged to apply)ASAP experience strongly preferredMust be comfortable working independently in the fieldReliable transportation and willingness to travel dailyStrong assessment and documentation skillsBilingual Spanish preferred, but not required
