POSITION SUMMARY - RN Case Manager


POSITION DUTIES - In an acute rehabilitation setting, coordinate and oversee each patient's individualized care plan to ensure safe, efficient, and goal - directed recovery from admission through discharge.


MINIMUM REQUIRED QUALIFICATIONS -

• WA/compact RN license
• BLS
• 2+ years recent acute hospital case management experience
• Utilization Review experience
• Ability to support team and family conferences for acute rehabilitation (facility will assist/train to this IRF Team conference requirement)

PREFERRED QUALIFICATIONS -

• CCM, ACM, ARN, RN-BC-CM
• 2+ years recent acute rehab case management experience

LENGTH OF ASSIGNMENT - 12 weeks


SHIFT / HOURS PER WEEK - 8a-4:30p M-F. EOW possible.


SYSTEMS - UDS PRO, Meditech, Pinpoint


START DATE - ASAP
