Patient Navigator/Administrative Assistant
An immediate need exists for a Patient Navigator with experience supporting underserved communities, particularly individuals affected by substance use and addiction. The ideal candidate will have experience scheduling and booking patient appointments in Epic Hyperspace, along with front-desk and administrative experience.
The Patient Navigator provides non-clinical support, guidance, and assistance to patients and families as they navigate complex healthcare environments. The role focuses on improving patient access, satisfaction, engagement, and outcomes while serving as a communication liaison to understand and address patients' individual non-clinical needs, concerns, and barriers to care.
Key Responsibilities
- Conduct outreach and engagement with patients referred to the clinic.
- Support individuals seeking care and help facilitate access to treatment.
- Schedule and coordinate patient appointments using Epic Hyperspace.
- Identify and address barriers that prevent patients from accessing care.
- Serve as a resource, advocate, and coach for patients throughout their care journey.
- Assist patients with accessing health insurance, primary care, mental health services, social services, and residential treatment programs.
- Work closely with counselors and other members of the care team to support patients and develop appropriate treatment plans.
- Assist with patient documentation and administrative processes.
- Build trusting relationships with patients using a strengths-based, compassionate, and trauma-informed approach.
- Help ensure patient safety, engagement, and continuity of care.
- Collaborate with the broader team to improve patient satisfaction and quality outcomes.
- Provide front-desk and administrative support as needed.
Ideal Candidate The ideal candidate is mission-driven, compassionate, collaborative, and committed to patient-centered care for individuals who use drugs or are experiencing substance use challenges.
Preference will be given to candidates who:
- Have experience working with underserved or vulnerable populations.
- Have experience supporting individuals affected by substance use or addiction.
- Have experience with patient scheduling and appointment coordination.
- Have experience using Epic Hyperspace.
- Have front-desk, administrative, or healthcare coordination experience.
- Have strong communication and interpersonal skills.
- Understand harm reduction and low-threshold approaches to healthcare access.
- Have connections to, or experience working within, the local community.
- Bring relevant lived experience and/or strong knowledge of the communities served.
Schedule:
- MondayFriday, 10:30 AM7:00 PM.
