Remote Care Coordinator - Chronic Care Management

The role supports medical, behavioral, and social drivers of health through person-centered care plans, assessments, and care coordination with the internal team and community resources.
This remote-friendly, full-time role requires strong organization, clinical and non-clinical coordination, and the ability to travel up to 50% in the territory.
