Social Worker – Adolescent Health (Primary Care) The Department of Social Work Services at the Mount Sinai Hospital will serve at‑risk adolescents and their families in an integrated medical and mental‑health primary‑care setting at the Mount Sinai Adolescent Health Center (MSAHC). The role includes individual, family, and group psychotherapy, case management, crisis intervention, and health education. Provide short‑term individual, family and group therapy tailored to adolescents. Conduct clinical assessments for patients referred by medical providers, including those with a history of trauma, substance use, or psychiatric disorders. Consult with physicians, psychiatrists, and other interdisciplinary team members to develop treatment plans and monitor progress using validated instruments. Offer transgender and gender‑nonconforming youth specialized support, including assessment for gender dysphoria and transition‑related counseling. Coordinate referrals and collaborate with external resources to meet client needs. Maintain documentation in compliance with New York State regulations and Mount Sinai policies. Participate in ongoing quality‑improvement initiatives to foster a youth‑centric, culturally competent care model. Qualifications NYS LCSW or LMSW license. Experience working with low‑income, inner‑city, HIV‑positive, or at‑risk adolescents. Proficiency in developmentally appropriate therapeutic modalities (e.g., TF‑CBT, CBT, MBT, DBT, EMDR). Demonstrated ability to work with complex trauma histories and LGBTQIA+ populations. Strong collaborative, clinical and communication skills. Excellent organizational and documentation capabilities.
