About This Role: The Clinical Care Manager leads a small team of Registered Nurses and Certified Nursing Assistants who provide advocacy, care coordination, wellness monitoring, and individualized support for Catholic Sisters residing within a retirement community.

The Clinical Care Manager consults and advises regarding healthcare needs and services, performing all essential functions of care coordination. This position contributes to the enhancement of the Sister's understanding of her own medical needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Assists and advises individual Sisters who have requested assistance or who have been identified as needing assistance with healthcare and related services.

2. Performs all essential functions of care coordination which includes assessment, development of health and related needs-based plan, implementation, monitoring, evaluation, and adjustment.

3. Supervises and delegates to Clinical Care Coordinators, CNA's and other health care team members appropriate care coordination duties.

4. Keeps Province Director of Healthcare Services and those appropriately involved in a particular Sister's health situation updated on a regular basis.

5. Works closely with Assistant Director of Healthcare Services in areas relating to insurance, benefits, and special equipment.

6. Maintains positive, professional working relationships with physicians, nursing, social services, pastoral, and other staff at Mary's Woods to facilitate optimal care of Sisters.

• works integrally with and as a resource to SNJM Community Life Enrichment staff.
• attends Mary's Woods care conferences weekly.
• attends weekly SNJM care team meeting.

7. Ensures coordination of medical appointments and transportation for Sisters living in Mary's Woods and off campus, as needed.

8. Directs non-SNJM Healthcare Proxies/Designated Powers of Attorney for Healthcare (DPOA) by educating the DPOA in end-of-life decision making and their responsibility in supporting Sisters' end of life wishes; assisting the DPOA with information and emotional support to help with healthcare decisions; and communicating regularly with the DPOA regarding any requested or necessary changes or adjustments to Sisters' care.

9. Assists in transitioning Sisters', who have declining or changing health conditions, from the various areas of Oregon to Mary's Woods or other appropriate placement location, including moving Sisters from one level of care to other levels in same facility.

10. Assists Sisters in communicating with healthcare providers (physicians, discharge planners, home health agencies, hospice, and healthcare facilities, etc.). As appropriate is in direct communication and coordination with physicians, discharge planners, home health agencies, hospice, and healthcare facilities, etc.).

11. In collaboration with Assistant Director of Healthcare Services, facilitates, coordinates, and monitors use of home care and other community-based services.

12. Transports and accompanies Sisters to medical appointments when health advocacy is needed, or supports others in doing so, especially at times of health transition, surgery, etc.

13. Serves as a resource to Sister and her chosen support person(s), especially during times of health transitions, serious medical conditions, and/or surgery.

14. Provides and/or assists with education of Sisters about health and wellness topics and available resources including planning, presenting, and/or implementing health and wellness programs (e.g., fall prevention, brain fitness, etc.)

15. Actively participates in periodic Health and Well-Being meetings, intercommunity health care meetings and workshops/seminars related to care coordination.

16. Documents Sisters progress notes/updates in Excel database tool.

17. Provides direct nursing care services on a limited basis with the approval of the Director of Healthcare Services.

QUALIFICATIONS: To perform this job successfully individuals must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or ability required. Education: Graduation from an accredited nursing program; BSN preferred.

Experience: RN with 10+ years management and care of older adults' experience

or equivalent.

Certification: Unencumbered RN License - State of Oregon

6. Demonstrated knowledge of and experience coordinating community resources throughout state of Oregon.

7. Supervisory experience; ability to coach, direct and lead staff in collaborative and respectful manner.

8. Ability to develop and maintain collaborative relationships with SNJM Sisters, staff, and external contacts.

9. High degree of flexibility and adaptability to shifting priorities and time constraints.

10. Excellent organizational skills and ability to prioritize and execute tasks in a timely manner.

11. Demonstrated written and verbal skills to ensure effective communication.

12. Ability to respond to urgent and/or stressful situations appropriately and calmly.

13. Proficient knowledge of Microsoft Office and other computer software programs including, but not limited to Word, Excel, Outlook.

14. Valid drivers' license.

15. Ability to travel frequently within Portland metro area on a daily basis in carrying out job responsibilities. Occasional travel throughout Oregon and other locations as needed.

PHYSICAL DEMANDS AND/OR WORK ENVIRONMENT:

Position requires work in office and residential settings, along with travel to conduct site visits to Sisters' homes and/or medical facilities. Must be able to assist Sisters with transportation, and with getting into and out of vehicles. Requires ability to lift/maneuver wheelchairs, walkers, and other medical equipment. Ability to lift 30 lbs.

HOURS : Full time. Occasional evening or weekend hours as needed. On-call every third week annually. May work 1-3 holidays annually. Occasional overnight travel.
Pay: $122,000.00 - $126,000.00 per year
