Position Overview

As an LPN or RN on this case, you will provide 1:1 nursing care to a middle-aged adult in their home. This is a great opportunity for a nurse who enjoys relationship-based care, consistency, and having the time to truly focus on their patient's needs.

Key Responsibilities

• Provide individualized skilled nursing care according to the patient's plan of care
• Administer medications and treatments as ordered
• Monitor and document the patient's condition and any changes
• Assist with medical equipment and other skilled nursing needs as appropriate
• Communicate with the patient, family, and Castor clinical team
• Complete accurate electronic documentation
• Provide compassionate, respectful care while promoting the patient's comfort, safety, and independence

Qualifications

• Current Illinois LPN or RN license
• Current CPR certification
• Strong communication and documentation skills
• Dependable, compassionate, and patient-centered
• Private duty, home health, or complex-care experience is a plus, but training can be provided based on the needs of the case
