LTSS Care Coordination Specialist Travel Ready
Molina Healthcare of Illinois
At Molina Healthcare, Inc. we seek a Care Manager focused on LTSS, coordinating care across settings to meet member goals. You will perform assessments, develop and monitor care plans, and collaborate with a multidisciplinary team to deliver high-quality, cost-effective services.
This role requires 2+ years in healthcare with LTSS or care management, licensure as applicable, and willingness to travel 25–40% locally. Strong communication and organizational skills are essential for success.
