Location 100 OLD BILLERICA RD,Bedford, MA, 01730,United States Employee Type Exempt - FT Required Degree Masters Manage Others No Contact information Phone View phone number on click.appcast.io Essential Duties and Responsibilities: Functions as a nurse practitioner within the scope of practice established by guidelines and protocols with the Medical Director, the Primary Care Physician, the Clinic Nurse Practitioner, the Director of Nursing and Carleton-Willard Village (CWV) policies and procedures. Works with Director of Nursing and Clinical Team at the Nursing Center to provide medical/clinical services in the SNF including regulatory visits as required and urgent care of medical needs as they arise. Review all labs, diagnostic studies and consultant recommendations and provide necessary orders and follow-up. Reviews and approves orders for new admissions and notifies attending physician of new admission. Participates in interdisciplinary team and family meetings for resolution of complex care problems when indicated. Participates in discharge planning meetings as needed. Provides individual and family teaching and counseling as needed. Enters orders electronically through Matrixcare. Consults with Medical Director and Primary Care Physicians on specific cases or other matters, as needed. Participates in problem solving of medical/clinical issues with nursing and other clinical staff. Attend weekly Medicare meetings and monthly QAPI meetings. Supports the mission, philosophy and goals of Carleton-Willard Village. May provide services to the Residents of Llewsac Lodge, Apartments and Cluster Homes when covering for the Clinic Nurse Practitioner. Committed to working with residents and families to develop and implement strategies that promote person-centered care. Applies “5 Star” customer service approach in all aspects of job. Adheres to all company policies, procedures, and standards of conduct. Reports to the appropriate managers any known or suspected violations of policy and procedures, regulations or standards of conduct. Attends required in-services and training. Performs additional duties as assigned. Requirements Nurse Practitioner licensed in Advanced Practice by the MA Board of Registration in Nursing and credentialed by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC). Master’s degree in Nursing with education and training in Geriatrics. Minimum of five years of Nurse Practitioner experience, including two years in a geriatric setting. Excellent communication and organizational skills required.
