You will provide critical care coordination that acts as a bridge service, assisting clients as they transition between different levels of care. Drawing on your knowledge of assessment and crisis intervention techniques, you will work with seriously mentally ill individuals, those with substance‑related disorders, and emotionally impaired clients. Your responsibilities include planning individualized development training, conducting needs assessments, advocating for client rights and services, maintaining thorough documentation, monitoring progress, and coordinating appointments while fostering effective teamwork. Qualifications & Skills To succeed, you need strong knowledge of crisis intervention principles and supportive counseling techniques, the ability to conduct assessments and develop individualized treatment plans, and effective communication skills to build rapport with clients and families. Organizational abilities are essential for maintaining detailed records and coordinating care, and proficiency in computer software tools is required for smooth documentation and reporting. Required Qualifications Bachelor's Degree in Human Services such as Social Work, Psychology, or Counseling. One (1) year experience providing behavioral health services to adults with serious mental illness or emotionally impaired. Must be registered with the Board of Counseling to practice as a QMHP or be eligible to register as a QMHP‑Trainee within 30 days of employment. Valid Virginia Driver's License. Selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test, and employment reference checks.
