Work Environment

You will be working onsite, primarily in patients' homes, which allows you to provide individualized care in a familiar environment. Key Responsibilities

• Evaluate and assess the needs of patients recovering from surgery, injury, or illness within their home environments.
• Develop and implement personalized treatment plans that utilize various therapeutic modalities, including physical exercises, stretching routines, and manual therapy techniques.
• Monitor and document patient progress meticulously, adjusting treatment goals and plans as necessary to optimize patient outcomes.
• Educate patients, their families, and caregivers on the use of assistive devices and modified strategies for daily living to support independence and recovery.
• Stay current with industry best practices by participating in workshops, training sessions, and continuing education opportunities.
• Build and maintain professional relationships with other health care team members and communicate effectively regarding patient care.

Patient Population / Client Focus

Your role will involve working with a diverse range of patients, including children, adults, and seniors, each requiring unique therapeutic approaches based on their physical conditions. You will leverage your expertise to help improve mobility, alleviate pain, and significantly enhance overall quality of life for your clients.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is preferred; a Bachelor's degree may be considered.
• Possession of a valid state license in Physical Therapy.
• Strong verbal and written communication skills to effectively convey treatment plans and motivate patients.
• Proficiency in computer and word processing skills.
• A flexible and adaptive approach to working in dynamic home environments.

Preferred Experience

• While prior work experience is not required, any exposure to home health care or outpatient therapy can be an asset.
• Experience working with various therapies and modalities to address comprehensive care needs is beneficial.

Compensation and Benefits
