As a Physical Therapist at Spring Creek Healthcare Center, you will play a vital role in our mission to revolutionize care delivery and provide exceptional rehabilitation services to our patients. Our premier 179-bed nursing home in Perth Amboy, NJ, is seeking a dedicated and compassionate Physical Therapist to join our dynamic team.
About the Role:
As a Physical Therapist, you will work closely with our interdisciplinary team to develop and implement comprehensive rehabilitation plans for our patients. You will conduct thorough evaluations, set realistic goals, and provide expert treatment to promote optimal recovery and independence. Your expertise will be essential in helping our patients achieve their maximum potential, and we are eager to welcome a dedicated professional like you to our team.
Responsibilities:
• Conduct thorough patient evaluations to identify rehabilitation needs and develop individualized treatment plans
• Provide expert physical therapy services to patients, including therapeutic exercise, manual therapy, and education
• Collaborate with other healthcare professionals to ensure a comprehensive approach to patient care
• Monitor patient progress, adjust treatment plans as needed, and maintain accurate records
• Participate in quality improvement initiatives to enhance the overall quality of care

Requirements:
• Doctor of Physical Therapy (DPT) degree or equivalent
• Current licensure to practice physical therapy in the state of New Jersey
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
