JOB TITLE: Admissions Liaison

LOCATION: Serving Medford & Wakefield Hospital Systems / Supporting RegalCare Facilities

DEPARTMENT: Admissions / Marketing

REPORTS TO: Regional Director of Admissions / Corporate Sales Leadership

POSITION SUMMARY

RegalCare is seeking an experienced, results-driven Admissions Liaison to drive census growth and lead referral conversions across our nursing home network. Operating primarily within the Medford and Wakefield hospital systems , the Admissions Liaison serves as the vital bridge between hospital discharge planning teams and RegalCare facilities.

This role requires a unique hybrid of clinical referral evaluation , bedside patient assessments , and strategic healthcare sales/marketing . The ideal candidate possesses deep expertise in the skilled nursing facility (SNF) admissions process, sharp clinical acumen to screen patient referrals, and proven sales strategies to successfully convert prospective patients into admissions.

KEY RESPONSIBILITIES

1. Referral Conversion & Clinical Screening

• Review complex clinical referral packages (medical records, therapy notes, medication administration records, physician orders) to determine appropriateness for SNF placement.

• Conduct prompt in-person bedside assessments at assigned hospitals in Medford and Wakefield to evaluate prospective patients and engage with families.

• Manage the full admission trajectory—from initial referral intake through bedside consultation to warm handoff at the facility level—ensuring maximum referral-to-admission conversion rates.

• Collaborate closely with facility interdisciplinary teams (Directors of Nursing, Business Office Managers, and Administrators) to secure swift intake approvals and clinical capability confirmations.

2. Hospital Relationship Management & Business Development

• Build, cultivate, and sustain strong professional relationships with key hospital personnel, including Case Managers, Discharge Planners, Social Workers, Hospitalists, and Physicians.

• Serve as the primary RegalCare brand ambassador and liaison within assigned healthcare systems, positioning RegalCare as the preferred post-acute care provider in the region.

• Identify emerging referral opportunities, analyze market trends in the Medford/Wakefield corridor, and implement targeted sales strategies to outperform competitor facilities.

3. Sales, Marketing & Community Outreach

• Execute dynamic sales strategies that promote specialized clinical capabilities, rehabilitation programs, and high-quality care offerings of RegalCare facilities.

• Conduct structured educational meetings, presentations, and clinical updates for hospital staff to maintain top-of-mind awareness for RegalCare.

• Maintain up-to-date documentation on daily referral logs, conversion ratios, market intelligence, and outreach activities using company CRM software.

QUALIFICATIONS & REQUIREMENTS

• Education & Credentials: LPN, RN, Social Work degree, or Bachelor’s degree in Healthcare Administration, Marketing, or a related field strongly preferred.

• Experience:

  • Minimum 2–3 years of experience in skilled nursing facility (SNF) admissions, discharge planning, or hospital-based liaison roles.

  • Proven track record of successful healthcare sales, referral conversion, and hospital network development.

• Core Competencies:

  • Clinical Aptitude: Proficiency in reading, interpreting, and evaluating clinical medical records and discharge plans.

  • Sales Strategy: Skilled in consultative selling, objection handling, relationship management, and closing prospective admissions.

  • Admissions Process Knowledge: Comprehensive understanding of Medicare, Medicaid, HMO/Managed Care insurance authorizations, and SNF regulations.

  • Communication: Exceptional verbal, written, and interpersonal communication skills to effectively engage hospital clinicians, patients, and families during sensitive health transitions.

• Mobility: Valid driver's license and reliable personal transportation required for daily travel between targeted hospitals in the Medford and Wakefield areas.
