Provide proactive, relationship‑based care focused on prevention, early intervention, and long‑term health outcomes. Coordinate follow‑up care, care management outreach, and continuity of care services to support patient progress and health goals. Counsel patients on medications, supplements, lifestyle interventions, and potential side effects or interactions. Order, perform, review, and interpret diagnostic testing including labs, imaging, and point‑of‑care testing. Assess patient history, symptoms, diagnostic results, and social determinants of health to develop individualized treatment plans. Diagnose, treat, and manage acute illnesses, injuries, infections, episodic concerns, and common chronic conditions across in‑person, virtual, and employer‑based settings. Collaborate with specialists, behavioral health providers, pharmacies, employer partners, and care teams to support management of complex or high‑risk conditions. Develop evidence‑based treatment plans aligned with HealthBar care models and employer‑sponsored health initiatives. Incorporate lifestyle medicine strategies including nutrition, movement, sleep, hydration, breathing, and mindfulness into patient care. Support employer wellness initiatives, health coaching, engagement programs, and preventative health efforts. Recommend and coordinate diagnostic, therapeutic, pharmaceutical, behavioral, and specialty interventions with consideration for safety, accessibility, cost, and effectiveness. Prescribe medications and therapies in accordance with clinical appropriateness, safety standards, and prescribing authority. Perform primary care and occupational health procedures including wound care, splinting, immunizations, medication administration, drug and alcohol testing, and workplace injury care. Conduct preventive and occupational health visits including wellness exams, sports physicals, DOT exams, pre‑employment screenings, and pre‑surgical evaluations. Coordinate referrals and continuity of care with specialists, imaging providers, therapy services, pharmacies, and ancillary vendors. Collaborate with interdisciplinary clinical, operational, and employer teams to improve patient outcomes, reduce healthcare costs, and support whole‑person care.
