Registered Nurse PD
Treasure Coast
RN PD Nights 8pm - 8am or RN PD Days 8am - 8pm
The RN is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with Treasure Coast Hospice policies and the Florida Nurse Practice Act, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To provide a combination of skilled professional nursing tasks and provide palliative and supportive care to the patient and family unit. Includes all areas of Clinical Support Services coverage; phone triage (call center) and in-person visits (responder), crisis care and provides coverage to the inpatient units as needed and assigned. Provides care in Martin, St Lucie and Okeechobee counties.Qualifications
- Current and active Florida RN license.
- Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
- Current and valid Florida driver's license and automobile insurance.
- Minimum of one (1) year RN experience with acute care experience, preferably in ER or as a Charge Nurse.
- Prior experience in oncology, home health, Hospice or palliative care setting is preferred.
- Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
- Must be able to effectively plan, organize and prioritize work.
- Must be self-directed and able to work independently in the field.
- Ability to read, write and speak English. Bilingual preferred.
- Excellent communication and interpersonal skills
- Basic computer skills and EMR software experience preferred.
- Must hold a current (BLS) CPR certification.
- Attendance is a condition of employment
- Evaluation after 90 days and annually thereafter based on competency review, chart audits and direct oversight.
- Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the Treasure Coast Hospice's Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.

- Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
- Provide comprehensive information to callers, and assist with emergent visits when needed, initiate/cover crisis care cases, case management coverage visits, transition alert visits, assistance with admissions, crisis care planning and any other visit types deemed necessary for the safe, effective provision of patient care as needed including back up staffing to the IPU and CNA visits depending on census and TCH care needs.

- Perform comprehensive assessments to include the physical, emotional, spiritual and psychosocial needs of the patients and family unit.
- Communicate to the IDT all pertinent information pertaining to the patient and family unit, actively to participate in review and revision of patients' plan of care.
- Documentation should include but not limited to variance in patient condition, interventions and outcomes and address progress towards goals set by the patient's plan of care.
- Based on verbal assessment determine the needs of the patient and family member.
- Assigns (call center/triage) staff based on the need.
- Knowledge of commonly used medications used with the Hospice patient.
- Documentation should include education provided to the family and the patient, including topic, which was taught and their understanding of the education.
- Makes patient visits to wherever the patient calls home; as indicated by need.
- Initiate communication with physicians and other staff members to coordinate optimal care and use of resources for the patient/family.
- Teach appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and others as appropriate. Support, instructed and educate the patient, family, and caregiver.
- Anticipate, prevent and treat undesirable symptoms or secondary symptoms.
- Assist in determining the need to change the level of care being provided for hospice patients based on assessment and collaboration with attending physician and/or hospice medical director.
- Provide appropriate support at the time of death, including disposition of medications, notification to funeral home, etc. according to agency policy.
- Maintain documentation in the EMR relating to care coordination including phone calls, communication, visits, and other pertinent clinical information.

- Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.
- Performs other duties as required and conforms with and abides by all policies and procedures.
- Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.
