Schedule: Monday-Friday - 8a-5p with 1 hour unpaid lunch

Clinical Details

EMR System: Cerner
Practice Settings: Clinic, no trauma/not rated
Admissions: 6-12
Support Staff: MA, RN
Work within interdisciplinary teams to maximize patient's quality of life by managing complex social needs, coordinating with doctors, and supporting families during serious illness
Job Duties: Psychosocial and Emotional Support, goals of care facilitation, Family counseling and education, Interdisciplinary Collaboration, Non-Clinical Care Management; focus on managing complex chronic conditions, preventing age related disabilities, and coordinating care to maximize patients' independence and quality of life
Patient Population: Geriatric

Requirements

Board: Board Certified in Internal Medicine
Active DEA - highly preferred
Fellowship training in aging - highly preferred
Licenses: Active AZ license or IMLC (LOQ in hand)
Malpractice: Clean MP/background
Experience: Must have experience working with older adults, typically those over 65 years of age
