A skilled nursing facility in the Norwalk, Ohio area is seeking an experienced and compassionate Social Services Director to lead our Social Services department. This is an excellent opportunity for someone who is passionate about resident advocacy, discharge planning, and supporting residents and families throughout their stay.

Responsibilities

• Lead and manage the facility’s Social Services department

• Complete psychosocial assessments, care plans, and required documentation

• Participate in MDS assessments, including BIMS, PHQ-9, and applicable Care Area Assessments

• Coordinate safe and effective discharge planning and community resources

• Assist residents and families with adjustment, placement, and psychosocial needs

• Participate in interdisciplinary care plan meetings

• Support PASRR and other regulatory requirements

• Maintain compliance with state and federal regulations

• Work closely with nursing, therapy, admissions, and other members of the interdisciplinary team

• Serve as an advocate and resource for residents and their families

Requirements

• Previous Social Services experience in a skilled nursing or long-term care setting required

• LSW preferred, but not required

• Knowledge of MDS, care planning, discharge planning, and psychosocial assessments

• Familiarity with PointClickCare (PCC) is a plus

• Strong communication, organization, and documentation skills

• Compassionate, resident-focused approach

• Ability to work effectively as part of an interdisciplinary team
