This per diem opening is ideal for a dedicated Nurse Practitioner skilled in Home Health, eager to conduct comprehensive risk assessments for Medicare Advantage members throughout Leon County, FL, including areas such as Tallahassee, Woodville, and Midway.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home annual health risk assessments for Medicare Advantage plan members
• Review and evaluate prior and current medical history, chronic conditions, and medication usage
• Offer education regarding medical conditions and prescribed medications to improve patient outcomes
• Identify care gaps and provide recommendations for follow-up, supporting ongoing patient wellness
• No responsibility for treatment planning or prescribing medications

Qualifications:

• Valid Nurse Practitioner License in the state of Florida
• Current CPR certification (BLS)
• Board Certification as a Nurse Practitioner through AANP or ANCC
• MSN degree from an accredited nursing program
• At least six months of recent Nurse Practitioner experience preferred
