Harmony Health Partners WA is seeking a Care Coordinator to support patients and coordinate care across clinical and community resources.
Position SummaryThe Care Coordinator plays a key role in helping patients navigate their care journey by coordinating services, supporting care plans, and improving continuity across settings. This position partners with internal teams and external providers to ensure patients receive timely, well-coordinated support.
In a hybrid work environment based in Yakima, WA, the Care Coordinator will help assess needs, coordinate follow-up, and maintain clear communication with patients, families, and care teams to promote positive outcomes.
Key ResponsibilitiesCoordinate patient care by supporting care plans, scheduling follow-ups, and ensuring continuity between appointments and services
Communicate with patients, families, and internal/external stakeholders to confirm needs, next steps, and timelines
Assist with tracking and documenting care activities to support consistent care delivery and accurate records
Identify gaps in services and help route patients to appropriate resources and supports
Collaborate with care team members to support care plan updates and streamline workflows
Experience in a healthcare or service coordination role, patient support, case management, or a related capacity
Strong communication skills with the ability to build trust and work effectively with diverse stakeholders
Ability to manage multiple priorities while maintaining accuracy and follow-through
Comfort working with confidential information and adhering to established procedures
Flexibility to work in a hybrid environment based in Yakima, WA
Familiarity with care coordination processes, care transitions, and common healthcare workflows
Experience supporting patients through complex care needs or navigating community resources
Demonstrated ability to collaborate across clinical and non-clinical teams
Experience with documentation practices that support continuity of care
Monthly salary: $4,200.00
