Job Summary and Responsibilities As our Cardiovascular Program Specialist, you will help patients achieve the best possible cardiac outcomes. Every day, you will serve as a vital resource to staff, the multidisciplinary team, and the community caring for our patients. You will apply your knowledge and skills in the care of patients with end‑stage disease, playing a key role in their holistic evaluation. Your strong critical‑thinking, interpersonal communication, conflict management, problem‑solving, autonomy, and decision‑making skills will be essential. Responsibilities Coordinate the referral process and introduction of patients for evaluation and assessment. Receive incoming referrals from physicians and hospitals and respond quickly to facilitate rapid access to the system. Actions include: contact with referring providers; review of pertinent clinical records; coordination of record review with multi‑disciplinary team members. Work collaboratively with the Program Medical Director and multi‑disciplinary team to schedule evaluation and procedure. Triage referrals according to written criteria to assure patient needs are met at an appropriate priority level. Communicate back to referring physician and/or hospital the plans for acceptance into the program. Educate the patient and family regarding the referral process, steps of evaluation, and keep patient and family informed throughout the entire process. Job Requirements Required Bachelor's of Nursing or equivalent work experience and 5 years cardiovascular experience. Evidence of caring for acute and chronic heart and vascular patients. Registered Nurse: AZ. Preferred Previous experience with VAD, ECMO, cardiovascular surgery.
