Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from a program accredited by APTA or equivalent.
• Current, active, and unencumbered New York State Physical Therapist license required prior to start.
• BLS/CPR certification from AHA or ARC required.
• Minimum 2-3 years of recent physical therapy experience preferred. (Preferred)
• Experience providing in-home therapy or working in Home Health settings required.
• Experience supervising and collaborating with PTAs and home health aides preferred.
• Ability to perform patient evaluations, develop and update plans of care, and document patient progress in accordance with physician orders and state regulations.
• Reliable transportation, valid driver’s license, and agency-required liability insurance for home visits.
• Proficiency with basic computer skills including electronic documentation, email, and online training platforms.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Evenings.
• Shift Pattern: 5x8.
• Assignment Type: Travel, temporary help.
• Start/End Window: Assignment runs from mid-May through mid-August (assignment length provided by client).
• Clinical Oversight: Provides skilled physical therapy per physician plan of care and may supervise PTAs and home health aides as assigned.
