Home Health Physical Therapist Work one‑on‑one with patients in their homes to help them maintain strength, independence, and quality of life in the activities they value. Responsibilities Provide individualized physical therapy services in the patient’s home setting, assess functional status, develop and implement treatment plans, document progress, coordinate with interdisciplinary team, and educate patients and caregivers.
