Acute Care RRT Navigator

Position Overview Acute Care RRT Navigator role within a top-tier Respiratory Therapy program at a three‑time Magnet‑designated hospital known for clinical excellence, high‑acuity care, and a culture of innovation. The position supports a patient‑first environment focused on respiratory care and COPD outcomes, partnering with nurses, physicians, APPs, pharmacists, case managers, and social workers. Responsibilities Collaborate with nursing and interdisciplinary teams to deliver comprehensive COPD education using written materials and visual aids. Coordinate referrals to pulmonary rehabilitation and pulmonary function testing. Serve as a liaison to clinical teams to optimize length of stay, safe discharge planning, and post‑discharge care. Conduct post‑discharge calls to reinforce action plans, medication adherence, and follow‑up. Teach lung anatomy, physiology, COPD pathophysiology, medication classes, exercise and pulmonary rehab, smoking cessation, oxygen and home equipment, and COPD‑focused nutrition. Perform the full scope of inpatient and outpatient Respiratory Therapy across the lifespan. Qualifications Graduate from an AMA‑approved respiratory therapy program. Active New York State Respiratory Care License. Registered Respiratory Therapist (RRT) credential. Preferred credentials: AARC COPD Educator and Tobacco Cessation Specialist.
