Job Summary Berlin Health & Rehabilitation Center in Barre, Vermont is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality patient care to our residents, working closely with our interdisciplinary team to deliver exceptional care and support. Responsibilities Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals Monitor and report patient progress and any concerns or issues that arise Maintain accurate and detailed records of patient care and treatment Participate in quality improvement initiatives to promote positive patient outcomes Demonstrate excellent communication and interpersonal skills when interacting with patients, family members, and healthcare team members Requirements Current LPN license in the state of Vermont High school diploma or equivalent required Ability to work flexible hours, including evenings and weekends, as needed #J-18808-Ljbffr
