Pathways to Success is an evidenced-informed model of intensive care coordination to support youth through family engagement; individualized care planning; and the utilization of strengths and natural supports. DuPage County Health Department serves as a Care Coordination and Support Organization and operates as a hub for care coordination for Pathways-enrolled individuals within DuPage County.An individual in the Care Coordinator position will be responsible for providing care coordination and support services to clients enrolled in the Pathways to Success program. The position involves working collaboratively with youth, their families, and community providers to achieve client goals.This full-time position works Monday through Friday, 8:00 am – 4:30 pm, with availability to work at least one evening a week, 10:30 am – 7:00 pm. The hiring range for this position is $44,400 to $52,000 depending on experience.This position comes with an outstanding benefits package designed to support your personal and professional well-being. Plus, you may qualify for the Federal Student Loan Forgiveness Program, making your financial future even brighter.ResponsibilitiesProvides care coordination and support services to Pathways-enrolled youth and their familiesManages a caseload of clients as defined by the Pathways to Success ProgramMaintains weekly contact with clients/familiesDevelops child and family teams for clients enrolled in the program and facilitates regular team meetingsProvides homebased and community-based services to engage at risk clients and support client self-management of their illnessDevelops and maintains client treatment plansParticipates in all required HFS planning, training, and reporting activities.Works collaboratively with community providersUse of an Electronic Client RecordDemonstrates independent judgment and discretion in making position related decisionsParticipates in emergency response activities as assignedRequirementsCompletion of a bachelor's degree in Psychology, Social Work, or a related field and one year of behavioral health experience; or an equivalent combination of training and experience. A valid Illinois Driver's License is required as this position will work within the community.
