Job Summary As a Patient Care Navigator (PCN), sometimes called a Community Liaison or Community Marketer, you will strategically grow our business within your assigned territory, enhance existing partnerships, and maintain connections with patients and their families. Your role includes: Identifying and cultivating new referral opportunities for Atlas Medical, Aegis Home Health, and Aegis Hospice. Delivering compelling Home Health and Hospice presentations at community networking events. Achieving monthly goals while maintaining high service and relationship standards. Handling related duties to support team success, demonstrating adaptability and problem‑solving skills. Desired Skills & Qualifications Minimum of 2 years Hospice Referral Relationship building and/or Marketing experience. Knowledge of the local senior care industry. Intuitive, solution‑oriented, and goal‑driven mindset. Detail oriented, flexible, and self‑motivated. Excellent oral and written communication skills. Hospice clinical background is beneficial. Valid driver’s license with a clean record and reliable transportation. Proof of current auto insurance, vehicle registration, and a valid AZ Fingerprint Clearance Card.
