Field Licensed Vocational Nurse

This role supports patients in safely managing their conditions while maintaining their independence and quality of life. Responsibilities Perform skilled nursing visits in patients' homes Provide nursing care according to physician orders and the established plan of care Administer medications and treatments within the LVN scope of practice Monitor patients' conditions and promptly report significant changes to the supervising Registered Nurse or physician Educate patients and caregivers regarding medications, treatments, safety, and care instructions Communicate regularly with the supervising Registered Nurse and interdisciplinary care team Complete accurate and timely clinical documentation Follow infection-control procedures, agency policies, and applicable regulations Travel to patients throughout Fort Worth, Arlington, and surrounding areas Qualifications Active and unrestricted Texas Licensed Vocational Nurse license Current CPR or BLS certification At least one year of nursing experience preferred Home health, hospice, or palliative care experience preferred WellSky/Kinnser experience is a plus Strong clinical, communication, and time-management skills Valid driver's license, reliable transportation, and current automobile insurance Ability to work independently under the supervision of a Registered Nurse Ability to travel throughout the assigned service area Why Join Amcare Pro?
