Care Initiatives Hospice in Cedar Falls, IA is seeking an RN Case Manager to provide comprehensive hospice care in patients’ homes or other care settings. You will work with physicians and a multidisciplinary team to coordinate care that respects patients’ wishes and enhances quality of life. The role requires an active RN license, CPR certification, and hospice experience or willingness to obtain CHPN certification. Travel across multiple locations within your region is expected. #J-18808-Ljbffr
