Position Title: Licensed Practical Nurse (LPN) - Long Term Care (LTC)
Location: Glenwood Springs, CO 81601
Duration: 13 Weeks (Possible Extension)
Work Schedule: Variable Day Shift | 12-Hour Shifts (6:00 AM - 6:00 PM) | 36 Hours per Week
Pay rate: 45-50$/hr

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to residents in a Skilled Nursing Facility (SNF).
• Administer medications, treatments, and wound care as prescribed.
• Monitor residents' health status and promptly report changes to the RN or physician.
• Assist in developing and implementing individualized care plans.
• Accurately document patient care and medication administration using Point Click Care (PCC) .
• Collaborate with the interdisciplinary healthcare team to ensure quality resident care.
• Maintain compliance with infection control standards, facility policies, and state and federal regulations.
• Provide compassionate, resident-centered care while promoting safety, comfort, and dignity.

To be considered for this position, you should have: [Skills, Education, or Experience]

• Active Licensed Practical Nurse (LPN) license.
• Minimum of 1 year of Long Term Care (LTC), Skilled Nursing Facility (SNF), or Rehabilitation nursing experience preferred.
• Current BLS Certification.
• Experience with Point Click Care (PCC) charting preferred.
• Strong clinical, medication administration, and documentation skills.
• Ability to work 12-hour day shifts, including weekends and holidays as required.
