Title
RN Case Manager - Home Health

Schedule
Typical schedule is M-F, 8-5. On-call and weekend rotation.

Assignment Details
Guaranteed Hours: 40
Contract Length: 13 weeks

Requirements
- Current RN license
- 2 years of recent acute care, home care, or hospice experience preferred
- Knowledge of clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred
- HCHB experience is strongly preferred

Roles & Responsibilities
- Provide skilled nursing and care through a team approach, based upon systematic evaluation as authorized by a physician.
- Observe, assess, evaluate, and plan interventions related to patients' physical and psychosocial statuses, including changes in condition and responses to treatments.
- Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the plan of care.
- Ensure continuity of care through effective communication and collaboration with other disciplines and community resources.
- Maintain accurate and complete patient records in accordance with agency standards.
- Participate in quality improvement activities and team scheduling as necessary to meet patient care needs.
