Physical Therapist

VNA & Hospice of the Southwest Region is seeking compassionate and motivated Physical Therapists (PTs) to join our team in Bennington County. We are hiring for a full-time position working with adult patients in the home health setting.

POSITION SUMMARY

As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide skilled therapy services in the home, facility, or educational environment. Caseloads may include adult and/or pediatric patients depending on experience and interest.

Responsibilities include:

• Completing PT and OASIS assessments as appropriate
• Developing and implementing individualized plans of care
• Providing therapeutic interventions and caregiver education
• Monitoring patient progress and adjusting treatment plans as needed
• Collaborating with interdisciplinary team members
• Supervising PTAs, students, and home care aides when applicable
• Maintaining timely and accurate clinical documentation

QUALIFICATIONS

• Degree in Physical Therapy from an accredited program
• Current Vermont PT license
• Valid driver's license, ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.
• Home health or prior clinical experience preferred
