Speech Language Pathologist (SLP) – Home Health $5,000 Sign-On Bonus Full-Time | Monday–Friday, 8:00 AM – 5:00 PM Coverage Area: Westmont, Hinsdale, Oak Brook, Downers Grove, Brookfield, Geneva, St. Charles, Bloomingdale, Addison, Hillside Why Join Us Consistent weekday schedule (no nights) $5,000 sign-on bonus Autonomy in your day with strong team support Build meaningful, long-term patient relationships in the home Collaborative, interdisciplinary care model Mileage-based local territory What You’ll Do As a Home Health Speech Language Pathologist, you’ll provide patient-centered care that improves communication, swallowing, and overall quality of life. Evaluate and treat patients using evidence-based speech therapy practices Develop and adjust individualized care plans based on patient progress Educate patients and families to support independence and long-term outcomes Collaborate with nurses, therapists, and physicians to coordinate care Complete accurate, timely documentation to support clinical and regulatory requirements Participate in care planning, case conferences, and quality improvement initiatives Identify changes in patient condition and adjust treatment accordingly What We’re Looking For Master’s Degree in Speech-Language Pathology Active Illinois SLP license (or eligible) Completion of Clinical Fellowship Year (CFY) Minimum 1 year of experience (or strong clinical internship experience) Strong communication, documentation, and time management skills Ability to work independently in a home-based setting Valid driver’s license and reliable transportation Certifications BLS Certification (AHA) – required within 6 months of hire (if not already obtained) Work Environment & Physical Requirements Travel within assigned geographic area Work in patient homes and assisted living settings Ability to lift up to 35 lbs (with assistive equipment/team support as needed) Occasional weekends/holidays based on patient needs Our Commitment We are dedicated to delivering high-quality, patient-centered care while supporting our clinicians with the tools, flexibility, and collaboration needed to succeed. #J-18808-Ljbffr Aurora Health Care
