Chaplain/Bereavement Coordinator Hospice (Full time)

Bereavement CoordinatorThe Bereavement Coordinator is an individual who provides emotional, psychosocial, and spiritual support to the caregiver/family before and after the patient's death and will assist the caregiver/family with issues related to grief, loss, and adjustment.Allegiance Healthcare Hospice and Palliative Care operate this facility. Our Mission is to provide the highest standard of care to our patients and the entire community that surrounds each patient. We strive to holistically meet their spiritual, social, and clinical needs in the most peaceful, dignified, and comforting manner possible.Under the supervision of a qualified professional, and as part of an organized program for the provision of bereavement services, the counselor shall implement bereavement counseling in a manner consistent with standards of practice, services include, but are not limited to the following:Evaluation and assessment of grief counseling needs and risk factors associated with griefAssessment of family response to grief and loss issuesProvide bereavement information and referral services to the bereaved, as needed, by the POCProvide bereavement information support to hospice staff as neededAttend hospice IDT meetingsDocument bereavement services provided and progress of the bereaved on a clinical progress note to be incorporated in the clinical recordMaintenance of confidentialityMechanisms to ensure that the family and caregiver's choices regarding bereavement contact or services are honoredBereavement: Documented evidence of appropriate training and experience in the care of the bereaved received under the supervision of a qualified professional.Spiritual care/Chaplain: Documented evidence of appropriate training and skills to provide spiritual counseling, such as bachelor of divinity, master of divinity or equivalent theological degree or training from an accredited school or university. An individual may qualify as a spiritual counselor without said degree if he/she has documented skills to provide spiritual counseling and has received equivalent training and supervision from an individual who meets one of the above qualifications.The bereavement counselor shall annually obtain two continuing education hours relative to the bereavement counselor's role, including but not limited to the following topics:Death and dying culturesSuicideCompassion fatigueAnticipatory griefPatient survivorsGrief groupsGriefLossAdjustmentEthicsAdvanced directives and LAPOSTThe spiritual counselor shall annually obtain at least two hours of continuing education related to the following topics, including but not limited to: a. end of life care;Cultural religious practicesCompassion fatigueSuicideDocumentationEthicsGriefLossAdjustmentAdvanced directives and LaPOST.1. Has not been excluded from participating in Medicare and Medicaid programs.2. Maintains eligibility to participate in Medicare and Medicaid programs throughout employment3. Capable of providing emergency care as needed for any patient4. Maintains a neat, well-groomed appearance at all times. Keeps personal appearance appropriate for patient care.5. Ability and willingness to participate appropriately in a disaster or evacuation situation.6. The worker will be spending considerable time up and about in the facility doing various tasks per the job description.Bending/Stooping: Frequently. Worker will be bending or stooping to work with supplies, preparation of documents, filing, working with clinical records, etc.Lifting Handling: Occasionally/Handling will be lifting/handling supplies to do inventory, Workers restocking, ordering, filing and auditing of clinical records, etc. Range of weight: 1-50 pounds.Carrying: Occasionally. The worker will be carrying clinical records, manuals, supplies, etc. Range of weight: 1-50 pounds.Pushing/Pulling. Occasionally. The worker will be pushing/pulling on various supplies storing, moving, and taking inventory of supplies as well as with clinical records filing, auditing, preparing for each month, etc. Range of weight: 1-50 pounds.Balancing: Frequently: Worker will balance supplies to be stored, moved from storage to using area, etc. When moving records from active to inactive status or during auditing periods, workers will be balancing legal documents that are not secure.Pivoting/Turning: Frequently. The worker will be pivoting and turning to stock supplies, handle records, etc.Crouching Stooping: Frequently. The worker will be crouching/stooping to file records, working with supplies to restock inventory of supplies, etc.Kneeling: Occasionally. The worker will be kneeling to file records, stock and inventory supplies, etc.Reaching: filing administrative folders, records; nursing file folders.A. Greater than shoulder height. OccasionallyB.
