Director of Health Partnerships & Value-Based Care
South Side Healthy Community Organization
South Side Healthy Community Organization (SSHCO) seeks a Director of Strategic Partnerships to lead development and management of strategic alliances advancing population health and financial sustainability. The role negotiates with MCOs, FQHCs, ACOs, health systems, and other partners to expand value-based care initiatives. Reporting to the Vice President of Population Health, this position builds mutually beneficial relationships, monitors performance, and aligns partnerships with SSHCO's #J-18808-Ljbffr South Side Healthy Community Organization
