If you’re a people person who enjoys building trust, providing guidance, and making sure every client feels supported from the very beginning, we’d love to meet you!About the RoleThis is a hands-on, client-facing role that combines relationship building, in-home visits, assessments, care coordination, and administrative follow-through. You’ll work closely with our Scheduling, Recruiting, and Operations teams to help ensure each client receives the right support and a smooth transition into care.What You’ll DoConnect & Build RelationshipsServe as a warm, knowledgeable point of contact for new clients and families.Build trusting relationships with clients, family members, caregivers, and referral partners.Guide families through the admission and service-start process with empathy and professionalism.Communicate with Spanish- and English-speaking clients and families to ensure they feel heard, understood, and supported.Visit & AssessConduct SUP visits, client assessments, and in-home evaluations.Evaluate the home environment and identify client needs, safety considerations, and readiness for care.Develop an understanding of each client's preferences, routines, and individual needs.Coordinate & CollaboratePartner with Scheduling and Care teams to support seamless onboarding and care transitions.Communicate client needs and updates to the appropriate internal teams.Help ensure care plans and service information are accurately communicated and documented.Follow up with clients, families, and team members to address questions or concerns.Document & Follow ThroughMaintain accurate and thorough documentation of assessments, client needs, visits, and care plans.Handle sensitive client information with professionalism, discretion, and care.Participate in weekend on-call or scheduled weekend responsibilities as needed.What We’re Looking ForFluent in Spanish and English - requiredExperience in home care, healthcare, caregiving, senior care, or another client-facing service environment preferred.Compassionate, personable, and comfortable building relationships with clients and families.Confident conducting in-home visits, SUP checks, and assessments.Strong communication skills, both verbal and written.Organized and detail-oriented with the ability to manage multiple priorities.Comfortable handling sensitive and confidential information.Collaborative and willing to jump in wherever needed to support the team.Positive, professional, and solutions-oriented.Comfortable working independently in the field while staying connected with the broader team.Willingness to participate in weekend responsibilities as part of the role.Why Join Total Care Connections?At TCC, how we care for people matters just as much as what we do.
