Position Summary

The Physical Therapist provides comprehensive therapy services across the District, including inpatient care, Skilled Nursing Facility and Swing Bed. This role evaluates, plans, implements, and monitors patient care programs designed to enhance, maintain, or restore physical function. The Physical Therapist ensures patient safety, develops individualized treatment plans with measurable goals, documents progress and outcomes, and collaborates with physicians, allied health staff, and multidisciplinary teams to optimize care.
Key Responsibilities

Provide physical therapy services in inpatient, Skilled Nursing Facility and wing Bed settings.

Evaluate, plan, implement, and assess patient care programs to improve functional outcomes.

Develop and implement individualized treatment plans with measurable goals.

Ensure patient safety during all treatment sessions and training activities.

Maintain accurate and timely documentation of patient progress and outcomes.

Serve as a consultant to physicians and allied health professionals on physical therapy needs.

Collaborate with healthcare team members to optimize patient care and discharge planning.

Provide training and education to patients, families, and caregivers.

Participate in staff education and deliver presentations on physical therapy topics as requested.

Uphold standards of practice in compliance with state regulations and accreditation requirements.
