Lao Family Community Development, Inc. (LFCD) is a community-based non-profit organization established in Richmond, Contra Costa County, California, in 1980. Since then, LFCD has expanded its operations to Alameda and Sacramento counties with its headquarters located in Oakland, California.
LFCD delivers programs and services from seven locations in more than 35 languages. The organization provides community development, affordable real estate, workforce development, education, and human services that primarily support low-income individuals and families, refugees, immigrants, transitional-age youth, seniors, individuals with disabilities, and other underserved populations.
Job Summary: Under the direction of the program supervisor, this position will serve low-income Workforce Innovation Opportunity Act (WIOA) participants and Oakland Housing Authority Program Participants to gain job placements, as well as manage participants under the California Advancing and Innovating Medical (Cal-AIM) Initiative, which provides comprehensive care management to address both clinical and non-clinical needs.
The Lead Case Manager is responsible for overseeing the Oakland Workforce Development Board(OWDB) WIOA Adult Services and Oakland Housing Authority (OHA) Adult Employment Services case management services, ensuring high-quality workforce services delivery, preparing the reports, ensuring compliance with program requirement and successful participant outcomes.
This position is responsible for interfacing with local employers and training providers, external organizations, and the internal LFCD department, especially the America’s Job Center of California (AJCC) and Business Centers. The position’s duties include lead the project, organize quarterly job fair, outreach recruitment for employers and job seekers, provide orientation, intake-assessment, set up Individual Employment Plan (IEP), case management, On the Job Training (OJT), Individual Training Account (ITA) / Vocational Training counseling, job placement, retention services, assessing the participants health condition and coordinating care. Program activities and strategies include long-term career advancement for each participant; understanding the hiring and business development needs of each employer; the policy and compliance requirements of funders; and the many subtle factors that can influence each individual’s ability to thrive and advance in the workplace. Knowledge of the human services needs of the local job market is a plus. This position is required to rotate at OHA Offices and LFCD AJCC locations and reports to the Program Supervisor.
Roles and Responsibilities:
- Lead and manage two primary projects:OWDB WIOA and OHA Programs.
- Develop and maintain detailed knowledge of programs such as WIOA, OHA and CalAIM Program, leveraging partnerships to enhance participant outcomes.
- Conduct individual assessments to develop personalized care management and employment plans, addressing physical health, mental health, substance use disorders, oral health, social determinants of health, career goals, and job placement strategies.
- Maintain accurate case files and documentation, including participant enrollments, progress tracking, eligibility verification, and compliance with HIPAA and workforce development regulations.
- Provide outreach and engagement in the community, attending resource fairs, distributing promotional materials, and coordinating referrals to social services, workforce programs, and healthcare providers to achieve participants’ care and employment goals.
- Lead workshops, job orientation sessions, one-on-one and small group counseling, and educational programs focused on job readiness, healthy living, independence, and goal setting.
- Organize quarterly job fair at OHA site(s).
- Prepare and submit program reports
- Assist participants in job readiness activities and with accessing benefits
- Provide intensive services to participants including: intake assessment, certify and obtain supportive documents for enrollments into the program; develop Individual Employment Plan (IEP), identify career goals, prepare assessment, and make appropriate referrals; provide up to 12 months of job retention services.
- Perform tracking of participants, input data, daily log, monthly case notes, and all other activities in CalJOBS system and OHA internal tracking form.
- Community Support: Services to address members’ health-related social needs to improve health and well-being.
- Maintain strong networks with employers, community stakeholders, and service providers, ensuring program participants have access to career opportunities and supportive services.
- Identify job opportunities with reasonable wages and benefits, conduct employer orientations, maintain employer relationships, and facilitate On-the-Job Training (OJT) opportunities.
- Collaborate with local colleges, career centers, and training providers to facilitate participant enrollment in workforce programs and communicate with training providers to ensure ITA request is submitted in a timely manner
- Provide administrative and programmatic support to leadership, including Directors, Managers, and Site Supervisors.
- Perform other duties as assigned.
Requirements and Qualifications:
- A Bachelor’s degree is required with a concentration in social work, business communication, or a related field. If no degree, a minimum of 2 year of workforce development and case management experience working with the sensitive need’s population; Bilingual in Spanish is strongly preferred .
- Strong case management, organizational, experience in report writing and data tracking.
- Ability to manage multiple projects and meet deadlines.
- Flexibility to travel to different work sites and attend meetings.
- Self-starter, ability to work with minimal supervision; excellent communication, community relations, networking, and public speaking skills.
- Demonstrated ability to work with individuals, families, and women without discrimination towards people of diverse cultures, race/ethnicity, socio-economic positions, ages, religions, genders, physical/mental challenges/disabilities, and sexual orientations.
- Understand, explain, and apply complex local, state, and federal regulations, policies, and procedures.
- Able to travel to meet with service providers and participants; ability to work independently as well as part of a team; must have a flexible schedule on some evenings and weekends.
- Proficient computer skills in MS Word, Excel, PowerPoint, database management, and Safari/Chrome/Edge.
- Must have a valid California Driver's License, clean DMV record, and proof of current auto insurance and registration; must pass a background check.

While performing the duties of this job, the employee is regularly required to sit and work on the computer; use hands to handle or feel and talk or hear; and move objects up to 25 pounds.
