Community Health Navigator: Family Wellness & Access
OYC Miami
Overtown Youth Center is seeking a part-time Community Health Navigator to connect families with health services, provide culturally relevant education, and help navigate social supports in Overtown and surrounding neighborhoods. The CHN will build trusted relationships, conduct outreach, assist with access to SNAP/Medicaid, coordinate with clinics, and uphold HIPAA confidentiality while supporting grant requirements. #J-18808-Ljbffr OYC Miami
