Board Certified Health Coach

To empower patients on their wellness journey, the full-time Board Certified Health Coach will provide remote consultations, implement behavior change strategies, and support personalized care plans while collaborating with interdisciplinary teams. Key responsibilities Implement coaching techniques and behavior change solutions to improve patient outcomes Conduct video consultations and messaging with patients to support their wellness goals Collaborate effectively with cross-functional teams to ensure optimal patient-centered care Required qualifications Board Certified Health and Wellness Coach Proficiency in motivational interviewing and establishing SMART goals with patients Minimum of 2 years of experience as a health coach or wellness educator Demonstrated competence in digital care delivery or telehealth Strong communication skills and ability to manage a patient caseload effectively
