Project Coordinator, RHTP Population Health

Responsibilities
Support implementation of RHTP / EBI projects within existing chronic disease program areas providing flexible support based on project and program needs. Design and implement data collection systems to ensure the availability of data needed to evaluate the effectiveness of RHTP initiatives. Develop and implement methods to monitor and evaluate the effectiveness of RHTP / EBI initiatives and assess whether programs achieve desired results in an efficient and cost-effective manner. Research compile and analyze data concerning health programs and services and use findings to recommend improvements alternatives and modifications to the availability delivery and funding of services. Prepare reports and recommendations addressing identified program issues and potential solutions. Develop and deliver informational presentations at meetings workshops and seminars to explain RHTP / EBI chronic disease initiatives. Coordinate with relevant chronic disease programs and internal and external partners to support successful completion of funded projects. Provide project and administrative support to the RHTP Program Manager as needed. Perform other related duties as assigned.

Qualifications
Bachelors degree in Public Health Community Health Health Administration Health Sciences Health Education Public Administration Government Social Work or a closely related field. Outstanding organizational skills and demonstrated ability to multitask prioritize and remain flexible. Excellent verbal and written communication skills including the ability to prepare written and oral reports and presentations that communicate necessary information to internal and external partners. Ability to work independently and as part of a project team. Proficiency in Microsoft Office applications including PowerPoint Word Excel or Access.
